The Claim deadline for this Settlement has passed.

The Court held its Fairness Hearing on November 9, 2009, and entered a Final Judgment approving the Settlement on December 21, 2009.

Appeals were filed by several Settlement Class members, but all appeals have been successfully and finally resolved.

The deadline for submitting a claim for monetary benefits was August 12, 2009. In order to be eligible for monetary benefits, class members were required to submit a properly completed claim form and meet the other conditions approved by the Court. If you did not receive a settlement payment, you may not be entitled to monetary benefits. Not all settlement class members were eligible to receive monetary benefits in the Settlement.

If you submitted a timely claim form as a Former Cancer Policyholder, and you are subsequently diagnosed with or receive treatment for cancer prior to April 23, 2019, you should submit proof of the diagnosis or treatment within 90 days of such diagnosis or treatment to the Transamerica address that was listed on your claim form.
